While Minnesota’s AL licensure application form is not yet final and published, it is expected to be made public on or about May 1, 2021. That leaves providers wanting to become licensed assisted living settings with a matter of a few short weeks to complete their application in time to meet the June 1, 2021 deadline already in statute. The good news is that MN Statute 144G.12 contains a starter list of the items that will likely be required on the yet-to-be-published form so that you can start gathering information now.
