    The Assisted Living Bill of Rights available here includes both the Rights found in 144G and the notifications and contact information required to accompany these Rights. This version was updated November 2025 and is effective January 1, 2026. An assisted living facility must provide the resident a written notice of the rights under section 144G.91 before the initiation of services to that resident. Existing residents must also be provided with updates to the Bill of Rights.

    Providers may either print or provide an electronic copy to each resident/responsible party. Providers must also obtain written acknowledgment from the resident of the resident's receipt of the assisted living bill of rights or must document why an acknowledgment cannot be obtained. Acknowledgment of receipt is to be retained in the resident's record.
